          The most important component of good therapy is a good fit. Feel free to contact me via phone or email with any questions you might have before deciding whether or not to schedule a session.

          If you feel ready to make an appointment, you may do so by filling out the information fields below.

          Once your session time is set, download and print out the appropriate PDF Intake Forms below. Please fill out the forms and get them back to me via email, fax or snail mail BEFORE your first session. That will give me an opportunity to get to know a bit about you before our session and allow us to not spend your first session time filling out paperwork.

          If you are absolutely unable to get the forms filled out before your first session, let me know in advance so I can print them out. You can arrive a bit early for your session and fill out the paperwork in my lobby and in that way will not lose out on any valuable session time.
